AFTER 16:  A website developed by the Useful Websites Family Fund for young people and families.  It has information about opportunities and services after school, benefits, transport, employment, friends, health and more.

CONNEXIONS SERVICE:  A national support service for all young people aged 13 - 19 to help to prepare them for adult life.  Connexions can support people with learning difficulties and disabilities until they are 25.

CONTACT A FAMILY:  A UK-wide charity which provides support, advice and information for families with disabled children, including information on transition.

COUNCIL FOR DISABLED CHILDREN:  Provides a national forum for the discussion and development of a wide range of policy and practice issues relating to service provision and support for disabled children and young people and those with special educational needs.
